Also Steam sometimes fails to automatically download mod updates from the Workshop. So try unsubscribing, waiting a little bit while your local version of the mod is removed, and then resubscribing.
Sorry to bother you, but needs an update to 1.18.1.1
Because Paradox added a "House Relation level icon" on Characters’ house Coat-of-Arms at the character window, there is a need for an update. After subscribing your mod, the new relation level icons on the house CoA doesn't appear. Your mod is a "must" mod for CK3 but because of the new 1.18.1.+ update, not compatible with mods like "Hometowns" that touch the character window (window_character.gui), I think... Male form not changing is due to a known vanilla bug [forum.paradoxplaza.com] in the title customization window, introduced in v1.18, where only the adjective, but not the title, gets saved.
This mod uses title customization window as the basis for "Edit FoA" window, so unfortunately this vanilla bug manifests as Male form not saving when the FoA is edited.
Hopefully, Paradox will fix this issue soon, in the meantime here is a workaround.
As a workaround, after entering your FoA and saving changes, right click on the "Edit FoA" button in the character/title window and then left-click it, to open Edit FoA window again - at this point the male form you entered previously will be applied; after that exit the Edit FoA window without making changes.

Note however, that FoA customization is only available for landed rulers, so the button will currently not appear for landless adventurers. This is expected behavior.
If you're playing as a landed ruler but still not seeing the button, try the usual troubleshooting steps for any mod:
1. Verify game files via Steam
2. Unsubscribe from the mod, wait a minute, then resubscribe, let it download and re-add it to your playset via PDX launcher.

Assuming you're not running any other mods modifying window_title.gui, this will allow you to see this mod's "Edit FoA" button in the title window for your primary title - it has exactly the same functionality as the one in the character window.
Making and maintaining a separate compatch for each TC is a time commitment that I'm not willing to take on. Anyone who enjoys a particular TC and is familiar with basic CK3 modding is welcome to publish their own compatch between said TC and this mod. And I will be more than happy to promote any such compatches on this Workshop page.
Alternatively, one could convince a TC to insert compatibility hooks for this mod directly into their GUI code - this is expressly supported by this mod's consistent use of GUI types/templates and would nullify the need for a separate compatch. But this obviously requires cooperation from the TC side.
But yes, one would have to make such a mod themselves (or maybe commission it from someone) to gain access to such a feature. I'm not aware of an existing mod for that.

It can be possible to make a mod binded to character_flags, which used customizable_localization to change prefix-title (or/and holder) names, but only to prescripted ones, but not to any of you want
