Yes. If you want the full bonus, you will need to skip BOTH item rooms. Skipping 1 only adds +15%, while skipping both adds +30%.

So I was playing around with the mod and I immediately used the plangar command to just test it and first there was no item at all in the planetarium. After that, not even resetting the run or reloading the mod seemed to reset planetarium chance. It's stuck at 1% even when skipping item rooms. The extrachance variable seems to not increment properly for some reason but I'm not sure if that's the only problem.

The item not spawning is definitely fixed now. There still seems to be some issues with how the planetarium chance is calculated tho. It starts at 31% on the first floor and entering both treasure rooms will still make it increase when going to the next floor. I looked in the code and you seem to calculate it in some odd places so maybe just calculating it in POST_NEW_LEVEL would help? That's how I would do it. Adding to the base chance depending on how many treasure rooms were skipped, and then running the rng after calculating the new chance.
Also one small thing is that I'd really like some sort of sound to play with the message that pops up. Something like this one: SoundEffect.SOUND_SHELLGAME

And don't be so hard on yourself, any mod that makes greed mode more interesting and enjoyable is a good mod in my book. I hope my feedback helps out. :)

Same as Akahara above, i've found 3 planetariums in my greed mod runs so far but one of them didn't present any item.

That should have been fixed in the most recent patch, can you try again? If that doesn’t fix it, it might be caused by another mod.

Really? Okay, why don't you enable the mod, open the console and spawn Reap Creep. When the mod is enabled that boss just dies instantly, I'm serious.

Okay, I’ll look into this, but I still think that sounds crazy.

I did exactly that and, it worked perfectly fine. No one died instantly. Are you sure it this mod causing the problem.
