Not showing up in the menu
I tried unsubscribing and resubscribing, but this level is still not showing up at all under the "Steam Workshop" menu. Is there something I'm missing?

I have the same thing.
The thing is that when I start HC nothing appears, no chapter, nothing.
Maybe after a year someone will write something?

According to the description, This map pack currently does not work correctly using the old game UI. If you are using the old game UI, please switch to the new game UI before running the map pack for the optimal experience.
