Failure to understand.
I must be missing something extremely obvious here...

Got the mod, subscribed and enabled... Then what I do? The lack of instuction implies that its super easy but I am failing to understand this somehow.

Do we both need contrrollers, do we both start a new game or what? What do we actually do?

start a run hook up another controller to your pc, then press start then a or x on the controller

I have the same issue. when my friend presses start, there is a letter on the top right corner. But when he presses x he starts as a baby

Yes. I only had afterbirth a month ago and then I bought afterbirth +, but I dont think it has anything to do with that.

wait, do you have the mod enabled in the mods section of the main menu? if it's gray it's disabled if it's black it should work.

Ah, I gotcha. So yes, a control is 100% needed AND from what I can tell its not able to link two computers... I assumed it was like that, I am too used to games doing that. My bad! XD

Did I get any of that wrong though, just in case?

nah all you need is a xbox one or ps4 controller and maybe a gamepad.

try uninstalling it and reinstalling the mod

go to challenges then go to custom challenges there is true co-op config
run the challenge change some of the option press R and presto its working
