"Germs" are the Horde SFX that you hear with music enabled just before a horde appears from wherever they spawn in as a way to warn the player. This is different for every campaign aside from L4D1. (L4D1 is the same across all maps pretty sure) Special Infected have the same sort of warning SFX called Bacteria in the files.
