Disable all your mods and see if the mutation works, if it does then it's a conflicting mod issue. Sometimes the game won't outright say that a mod is having conflicts.

Yes, it should.
Since L4D doesn't have steam workshop support I doubt it.
Unfortunately, that's not currently possible with Source's limitations.

I had some extra free time today and was able to finish a Vscript version of the mutation:

So as of right now I don't think I can turn this into a Vscript, since they work differently. I'd have to basically change how the code works entirely to get it to work. I also can't be certain it'll work in multiplayer sessions. Plus in normal games without the mutation players will just respawn in closets.
You're more than welcome to look at the source code itself and see if you can get it to work. Also, if you just want to have it so realism rules aren't applied, you can download the mod and edit the modes .txt file so the base mode is "coop" and not "realism."
