It looks a little more fine on the SFM promos, but the lighting on the regular screenshots are hard for me to vote on this one. If it was made in HLMV, maybe it requires a better lightrot, as the colors look flat and too shiny.
Also I'm not the one who talks about team recognition, but this one I may agree. Although, I understand that Heavy's animations are much more different than Medic's, having a Heavy being pocket by a Medic might actually confuse some people (and I'm aware you can paint any cosmetic with his coat colors with the Waterlogged Lab Coat paint, but still).
He may be big, but if you see white lab coat with team-colored gloves, you could mistake him for Medic.
Hard no.
That being said, you can clearly model very well and work with these others to create items. I sincerely suggest you take a look at Valve's official guide to TF2 item creation and take some notes. I hope you end up pushing out a banger item
